Report Internal Fraud/Government Waste

Tooele City’s Fraud Hotline allows citizens and employees to report potentially improper activities by Tooele City employees such as:

  • Waste or misuse of public funds, assets, or manpower
  • Violations of law, rules or regulations applicable to the government agency
  • Abuse of authority
  • Unethical conduct
  • Neglect of Duty


How do I File a Complaint?

Submit complaints in writing using the outline detailed below.  Submit any additional evidence such as pictures with your complaint.  Complaints must be reliable, specific in nature and have credible evidence to be accepted.

Complaint Submission Outline:

  • Date of submission
  • Name or Anonymous*
  • Address
  • Contact phone number or email address
  • Responses to the following six questions
    1. What is the fraudulent activity?
                       –  Provide specific information detailing “who, what, when, and where”.
    2. When and where did the activities take place? Includes date(s), time(s), and frequency.
    3. Are there any other persons who may have additional information or who may have witnessed the event(s)? If so please provide their contact information.
    4. Is there evidence or documentation that can be examined? If yes please attach such evidence.
                       –  Provide credible information substantiating the complaint
    5. Would you like to be contacted with follow up or results of this complaint? If so, please indicate your desired method of contact

*If you wish to remain anonymous name and contact information is not needed
